Chronic Care Management (CCM)

Attention Medicare Patients

We are now able to provide our patients with Chronic Care Management (CCM). CCM is a Medicare-preferred service available to our Medicare patients living with two or more chronic conditions such as diabetes, high blood pressure, or kidney disease. It provides health support between office visits with personalized care coordination.

Patients in a CCM program are less likely to be hospitalized or need emergency room visits. The program can also help our patients save on out-of-pocket healthcare costs.

Your personalized Care Coordinator is an extension of our office here at Total Patient Care.

• Medication Reviews
• Alerts to Your Provider
• Ongoing Health Support
• Social and Community Resources

We are currently enrolling Medicare patients who also have Secondary Insurance.
